During the first visit, we make sure to obtain important background information, and give you time to get to know your doctor. Our friendly staff is dedicated to caring for you and your entire family's foot health and well-being. We take the time to create a personalized treatment for each patient and we offer a wide range of services. It is our goal to make your visit with us an extremely pleasant one.
Our Philosophy
Our office specializes in providing the highest quality of care of the lower extremities, including the foot, ankle and lower leg. Dr. Jessica Taub, Board Certified Podiatrist, is providing quality care to residents of Martin County and surrounding areas. Whatever troubles are ailing your foot, ankle or leg, we will do everything in our power to bring you the relief you have been seeking.
Patient Forms
Please print and fill out the New Patient forms so we can expedite your first visit.
New PatientsPlease arrive early so that we can complete your initial paperwork. Also, please bring the following:
- Patient's insurance card
- List of current prescriptions and/or over-the-counter medication, including dose and frequency
- Information about patient's medical and surgical history
- Recent test results, x-rays, or relevant records
Insurance and Payment Information
You are responsible for co-payments or charges that are not covered by your insurance. If you have questions regarding billing or which insurance plans we accept, please call our office. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
